Nursing care management for a stroke patient

Presentation: You inherit a new patient F. L. who has a history of “recent stroke.” He was recently (approximately 3 months ago) hospitalized, but you don’t have his records at the moment. The patient’s wife wanted him “checked out,” and states that his “cat scan” showed a stroke. She claims, “They did a bunch of other tests but didn’t find anything.” She does not think he has hypertension but adds, “He does not like to go to doctors.” The past medical history is unknown. He does report taking aspirin 81mg daily and does not have any known drug allergies. Patient F. L. smoked one pack a day but quit five years ago, however he does drink socially. As for his family history, his mother died at the age of 79 from a stroke and his father died at the age of 64 from a myocardial infarction; he is the only child. He is 57 years old, married to his wife for 29 years and has three children. He works Monday through Friday as an accountant.

Medications: Aspirin 81mg daily

Review of systems: Subtle left hemiparesis; increased tone and hyperactive reflexes; very mild neglect of his left side.

Physical exam: Vitals: height = 72″, weight = 225, body mass index = 30.5, blood pressure = 150/99 mm Hg; heart rate = 93 beats per minute at rest.

  • Describe the pathophysiology of a stroke.
  • What would your nursing assessment consist of? What should you be cognitive of with stroke patients?
  • What are three short-term goals for this patient?
  • What are three nursing interventions for this patient?
  • What are three things you would discuss with the patient and his family during your discharge teaching?
  • What are two medications (other than aspirin) that could be provided for this patient and why?
    • What is the mechanism of action of each?
    • What are some of the possible side effects associated with these medications?
    • How would the patient benefit from taking these medications?
  • What are three recommendations for follow-up care?

Part 1

Benchmark –   Capstone Project Change Proposal

In this assignment, students will pull together the capstone project change proposal components they have been working on throughout the course to create a proposal inclusive of sections for each content focus area in the course. For this project, the student will apply evidence-based research steps and processes required as the foundation to address a clinically oriented problem or issue in future practice.

Develop a 1,250-1,500 written project that includes the following information as it applies to the problem, issue, suggestion, initiative, or educational need profiled in the capstone change proposal:

  1. Background
  2. Clinical      problem statement.
  3. Purpose      of the change proposal in relation to providing patient care in the      changing health care system.
  4. PICOT      question.
  5. Literature      search strategy employed.
  6. Evaluation      of the literature.
  7. Applicable      change or nursing theory utilized.
  8. Proposed      implementation plan with outcome measures.
  9. Discussion      of how evidence-based practice was used in creating the intervention plan.
  10. Plan      for evaluating the proposed nursing intervention.
  11. Identification      of potential barriers to plan implementation, and a discussion of how      these could be overcome.
  12. Appendix      section, if tables, graphs, surveys, educational materials, etc. are      created.

Review the feedback from your instructor on the Topic 3 assignment, PICOT Question Paper, and Topic 6 assignment, Literature Review. Use this feedback to make appropriate revisions to these before submitting.

Prepare this assignment according to the guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is not required.

Module 2 medical ethics and law

Throughout this week, you read about managed care and third-party payers, the basic components of insurance.

Up until recent years the only options for the uninsured, if they qualified for it, were Medicare and Medicaid. With the passage of the Affordable Care Act, the landscape of insurance is changing.

For this week’s written assignment, read the following article:

Hamel, M.B., Blumenthal, D., Abrams, M., & Nuzum, R. (2015)

http://www.nejm.org/doi/full/10.1056/NEJMhpr1503614#t=article

.

The New England Journal of Medicine, 372, 2451-2458. Retrieved from

http://www.nejm.org

After reading the article, answer the following questions:

  1. What are your initial thoughts from the article?
  2. How do you think the Affordable Care Act improves health care for Americans?
  3. Who do you think will benefit the most from the Affordable Care Act? Why?
  4. Why are some states slower to participate than others?
  5. If you could create a healthcare plan for the United States, what would it include, who would be covered?

Creative Project – Final

Based on your research essay, you will construct a project that puts you in the time period and creatively tells a story from your research. It will be 7 pages total (unless you are doing a video – in which case it should be 1-3 minutes long.)

Suggestions might include:

  • Invitation to a lecture by a female scientist
  • Journal of a female astronaut
  • Mock trial of passing the Equal Employment Act
  • Recipe book for women in science
  • Employment ads seeking a female doctor
  • Yearbook from a woman studying science at university
  • Church bulletin for civil rights activists
  • Descriptive battle maps of French encounters
  • Letter back home from a black soldier
  • Pamphlet by NAACP outlining their beliefs and aims
  • Newspaper article describing discrimination of black soldiers
  • Tourism website for Houston in the 1920s
  • Social media account of a WWI recruiter
  • Twitter account by President Wilson

FINAL PROJECT

  • Written Description – In a one page written introduction, tell me why you chose this format and what you learned in the creative process. What software did you use to complete it? What historical examples gave you ideas for your project? (1 page, 20 points)
  • Creative Portion – The best part! Include images, clever formatting, and enough detail to prove your research. You can either upload a digital version or, if you have drawn by hand, take a photo and upload that. Save any digital versions (Word, Publisher, PowerPoint, whatever, as a PDF so I will definitely be able to open it). Videos are great, too! Or, create a website! (5 pages, 70 points)
  • Works Cited – Include whatever sources you use to inform your project. This might be the same as your bibliography, but you still need to include it. If you add or subtract any sources for the creative portion, make the changes on this works cited. (10 points)

Discussion: Psychological Aspects of Aging

Theories of successful aging explain factors that support individuals as they grow old, contributing to their ability to function. Increasing your understanding of factors that support successful aging improves your ability to address the needs of elderly clients and their families.

To prepare for this Discussion, review this week’s media. In addition, select a theory of successful aging to apply to Sara’s case.

Parker Family Episode 2 Program Transcript PARKER: Ever since my husband died, there’s been no one to talk to. It’s just, really, no one. And when Stephanie is home, I just feel so alone. FEMALE SPEAKER: What about the day center you go to? Isn’t that helping? PARKER: I don’t like it. What makes me really feel good, though, is when I go shopping, buying things. And my kittens. I love my cats. Oh, have you seen them? I have pictures. Just take a look. Look! These are so cute. My babies. FEMALE SPEAKER: Yes, they’re very cute. And wow, you have a lot of them.  PARKER: Oh, well, it’s their home, too, not just Princess Stephanie’s.  FEMALE SPEAKER: The day program you’re attending, are you seeing a  psychiatrist there?  PARKER: Yes. Dr. Lewin.  FEMALE SPEAKER: May I ask how that’s going?  PARKER: He says that I’m depressed.  FEMALE SPEAKER: In the pictures you showed me, you just talked about the  cats, but I also saw all the things you keep around you, the hoarding. I understand how depressed you been since your husband passed away. How alone you’ve felt. [SIGH] But I would like us to try and set up a plan to begin to address the hoarding. It’s very clear that that’s one of the big issues that’s affecting your relationship with Stephanie and your life together. Can we try that? PARKER: I don’t like it when we fight. She’s still my baby, too. Yes, I want to try.

Specifically, the following critical elements must be addressed:

  1. Writing Plan: Use this writing plan as a way to gather your thoughts and determine your strategy for writing your critical analysis essay. This process will allow you to develop a potential structure for effectively communicating and supporting your claim. This plan will be helpful in keeping your thought process on track when you begin writing and revising your essay.
    1. Describe the argument to be addressed in your persuasive essay. Explain how the argument relates to your major, the major you are considering pursuing, or your field of work.

    3. Identify the audience who will be reading your essay. What potential challenges will you encounter in supporting your argument with this demographic?
    4. Establish a goal that you hope to accomplish with this essay. What will this essay need to be successful?
    5. List potential resources that could be used as supporting evidence for your argument, and provide a brief description of each and how they will reinforce your argument.
    6. Align key points of your argument with supporting resources for establishing an effective essay structure.
    7. Based on your argument, determine at what points in the essay integrating evidence would be most effective, and explain why.
    8. Describe a revision strategy that would be most effective while writing this essay. Why would this strategy be effective?
    9. Review your writing notes assignment and the feedback provided by your instructor. How does this feedback influence your progress moving forward? What changes did you make to your writing plan after receiving this outside feedback?

Merit-based pay plan or an incentive-based pay plan

Imagine that you are the human resources (HR) manager at a company. The company you work for is trying to determine whether it would be better for its employees to be compensated using a merit-based pay plan or an incentive-based pay plan. As the HR manager, it is your responsibility to explain each pay plan to your company’s board of directors and voice an opinion about which one you feel would work best for the company’s employees. In addition, you must explain your choice.

Create a PowerPoint presentation in which you define each type of pay plan and explain the similarities and differences between them. Also, you will express an opinion about which pay plan would work best and why you feel this way.

Your presentation should consist of no less than 10 slides, not counting the title slide and reference slide(s). Any references used, including your textbook, should be cited and referenced using APA formatting.

Textbook: Martocchio, J. J. (2017). Strategic compensation: A human resource management approach (9th ed.). Hoboken, NJ: Pearson.
