Ambulatory Case Study

In the final project, students will apply the knowledge gained in this course to a case example.

Each student will select a SPECIFIC real Ambulatory Surgery Center and apply one of the scenarios below and develop a 10-12 slide PowerPoint presentation to discuss the current status of the organization, provide an overview of the organization, and make recommendations on how to manage the selected problem. You must include one or more slides that address the degree to which the Affordable Care Act might impact these services. Use APA formatting references and your reference slides. For this presentation, your title slide does not have to be in APA format.

SCENARIO OPTIONS. Pick one of the scenarios below. Your role is the manager in charge of an ambulatory surgery center (ASC). Restate the problem you are addressing. Make some logical assumptions. Make brief recommendations as to a reasonable solution to the problem in the scenario.

POINT OF CLARIFICATION:  You will use an established independent ASC for either of the four scenarios listed below.  Your presentation should be drafted as the administrator of the surgery center working to address one of the four scenarios to the CEO or Board of Directors of the ASC.  In your response, please be sure to address all components of the scenario.

  • One of the greatest challenges facing ASCs is maintaining and increasing volume. With all the external pressures, i.e. physicians pressured to become employees, mergers, decreasing payments from insurers, etc. – ASCs are challenged to keep physicians motivated to increase the number of cases and keep patients satisfied. What are some strategies you would use to increase referrals to your ASC while maintaining high patient satisfaction scores?
  • Another challenge facing ambulatory surgery centers is the recruitment of new physicians. Following a national trend, many local hospitals and health systems are purchasing physician practices, which could eventually lead to doctors referring surgical cases to a hospital’s same day surgery department. Assuming there is ample opportunity to increase surgical volume, how do you intend to recruit physicians to practice via your ASC? What are some strategies/offerings you could employ?
  • The ASC you are currently managing is experiencing out of network challenges. Additionally, the proposed reimbursement rates from payers do not match Medicare rates for your market. Most of your ASC contracts are based on the Medicare fee schedule and because many of your surgeries are not on that schedule, you have to negotiate a fee schedule on a case-by-case basis. What strategies could you employ as you work with payors to contract for reasonable rates that could save you both money in the short term?
  • In this changing environment, you are required to do more with less. Reimbursement rates are declining; supply and implant costs, taxes, utilities, rent, salaries and benefits and other operating costs are increasing and there are more regulatory requirements i.e., increased standards for accreditation (Joint Commission’s additional requirements as an example), ICD-10, adoption of an EHR, etc. Additionally, your staff is wearing multiple hats; many working to register, bill, code, schedule and assist with back house operations. What strategies and tools can you implement to increase productivity, reallocate resources and boost employee morale?

ECONOMIC

A central theme of our course is decision making.  My main goal is for you to see the concepts, models and theories of economics in the real world, so that you can use them to make better decisions in business and life. One of the tools we use throughout the course is marginal analysis: understanding how to find the “optimal” or “efficient” quantity of something by comparing costs and benefits at the margin.

Assignment:  Write an essay explaining what “marginal” means and explain why marginal analysis leads to optimal or efficient quantities. Use applications we learned in our course (consumption decisions and production decisions) to support the answer. Adding examples from life outside of the classroom is strongly encouraged.

In short, put together a comprehensive view of a topic that was covered throughout the course. I’m not looking for a laundry list of topics. Rather, you should write a cohesive, accurate and easy-to-understand essay that someone new to economics could use to understand the idea of “marginal” and how marginal analysis works to help us make better decisions.

    1. Which country, of the pair, is the most receptive? Provide a well-reasoned argument using the evidence presented in Projects Part A, Part B, the Economics and the Infrastructure parts of this project.

1) Phoenicians were Semitic people inhabiting ancient Phoenicia and its colonies. The reason of what innovation allowed the Phoenicians, a small population to change the history of the world, is because they had more ready access to valuable resources and trade routes, and the Phoenicians originally established the colonies as trading posts. These trading posts blossomed into full on colonies, and they were established in good harbors to further their duty not only to trade but also to providing rest for Phoenician traders. These posts were closer to the rich Mediterranean and all of its resources. This made it so that they were able to accumulate many resources and have easier trade with the rich trade centers of the west. While the Phoenician homeland was amalgamated into the Assyrian empire and its successor empires, the colonies continued on.

2) The reasons why Daoism and Confucianism reflect the diverse attitudes of the time is because, Confucianism is rites, righteousness, relationships and benevolence towards others. Also very important is keeping order in relationships and in the state. This is more of what you are controlling in you life. Daoism is the most mystical of the major schools. As the name suggests, the focus on the Dao, or the Way. Now, what is the Dao? There are several complicated answers to this, but most simply, the Dao is the natural flow of the cosmos. Nature follows the Dao, humans with their conscious wills go against the Dao. The goal of the Daoist is to harmonize with the Dao and thereby become one with the cosmos, with nature, with all things, which is for letting everything naturally flow.

The Phoenicians, who were described as a small group of unwarlike Semitic people, had several accomplishments such as becoming the greatest maritime traders and colonizers from their ports of Tyre and Sidon. They also specialized in trades of copper and dyes that took them as far as the coast of Britain where they were able to form bronze which was the main metallic resource before 1000 B.C.E. However, their most notable contribution of their time was inventing a phonetic alphabet consisting of 22 letters. They used their alphabet while operating their trade network and it spread throughout the Mediterranean. Once it spread to Greece, the Greeks added letters for the vowels and thereby created the same alphabet that we use today. 

The Warring States Period was an era in China’s history that was filled with turmoil and led to many political and social changes. During the Warring States period, many innocent people were being killed due to the battles taking place and there seemed to be no real value for human life. Confucianism and Daoism both came at a time when the people of China needed something to believe in, something to help guide them in their life choices and something to help them feel connected to others. Confucianism was a more practical philosophy and focused on family and relations to other people in their life. It has a lot of the same ideals of older-day, Leave-It-To-Beaver style American families in the sense that the father is head of household, the wife never talks back and the children and grandchildren show absolute obedience. His philosophy offered a very harmonious family style.  Daoism was described as a more skeptical philosophy focused on nature and meditation. It teaches it’s followers to live in the moment and not to fight anyone or force an outcome of any kind.

Review  the “AT&T Pulls $39 Billion T-Mobile Bid on Regulatory Opposition.” article.

In 2011, AT&T attempted a merger with T-Mobile. The Justice Department sued under the act, claiming that the merger would constitute a violation of the antitrust laws. In 2012, AT&T dropped its attempt at the acquisition.

If AT&T had merged with T-Mobile, would the merger have violated antitrust laws? Why or why not? Do not be unduly influenced by the Justice Department’s stance on the issue. Use your own analysis to reach a conclusion.
